Applications for public housing can be picked up at the administrative office located at 540 Harrison Street, printed out from our website, or can be mailed directly to you. You must return the application to us in person Monday through Friday between 8:00am and 4:00pm. The application must be completely filled out with correct information. Failure to do so could result in your application being denied. You must provide proof of family income, a picture ID for all family members 18 and older, social security cards and birth certificates for all family members. Your application will not be accepted without these documents. A QHA staff person will review the application with you and is available to assist you with any questions you may have. A criminal background check will occur for all adult household members.
You MUST be 18 YEARS OLD and have the following items with you to apply:
- Birth Certificates for all members of the household.
- Social Security Card for all members of the household.
- Photo I.D. for all members of the household 18 years of age and older.
- Proof of income for all members of the household if applicable.
SENIORS or QUALIFIED DISABLED:
- Proof of medical expenses (prescription drugs, supplemental insurance).
- Proof of unpaid medical bills (physician charges, hearing aid, hospital, eyeglasses, etc.).
- Checking and/or savings account bank statement (savings must show amount in account and current rate of interest).
- Proof of Social Security payment (annual award letter), Pensions, etc.
- Proof of all assets.

SOCIAL SECURITY NUMBERS WILL BE REQUESTED FOR VERIFICATION ON ALL INQUIRIES CONCERNING THE APPROVAL OR DENIAL OF THE PUBLIC HOUSING APPLICATION AND THE POSITION ON THE WAITING LIST
